Custom Guitar (Electric)?

That manufacturers have the option of building custom guitars? I know Fender, BC Rich, but what are the others??? And do you consider the best? I also want to know what the lowest price for custom guitar (approximately), I know it depends on gear you have on it, but I need the exact price. So help, please if you can. Thank you.
